Description : A support service for frail aged people who require a range of care services in their own housing. Care provision is developed and monitored through case management to ensure support for those at risk of admission to residential care.
Details : OFFICE HOURS: 9.00am to 5.00pm, Mon to Fri.

AVAILABILITY: Thirty Community Aged Care Packages.

REFERRAL: Aged Care Assessment Service.

SPECIAL REQUIREMENTS: Must be over 65 years old.

ASSESSMENT: Low-level-care community care.
